COLORADO COMMUNITY FIRST CHOICE
More support. More choice. More independence at home.
Community First Choice (CFC) is a Health First Colorado benefit that helps eligible children and adults receive authorized long-term services and supports in their own home and community. Colorado implemented CFC on July 1, 2025. Through CFC, members may be able to choose how certain services are delivered, including In-Home Support Services (IHSS).
WHAT CFC MAY SUPPORT
Personal Care
Help with authorized daily activities such as bathing, dressing, grooming, eating, mobility, and other personal needs.
Homemaker Services
Authorized help with routine household tasks such as light housekeeping, laundry, meal preparation, and shopping.
Health Maintenance Activities
Authorized routine health-related tasks provided according to the member’s assessed needs, care plan, and program requirements.
WHO MAY QUALIFY
Eligibility generally begins with enrollment in Health First Colorado, an assessed need for covered services, and meeting the applicable level-of-care and program requirements. A Case Management Agency completes the assessment, helps develop the person-centered care plan, and determines authorized services and hours.

FAMILY CAREGIVERS CAN FEEL VALUED
Depending on the member’s circumstances and program rules, a qualified family member, friend, or current caregiver may be able to provide authorized care and be paid through an approved agency. Leon Home Care offers practical onboarding, training, dependable payroll, and responsive support. Contact us for current caregiver pay information.
Eligibility, service authorization, caregiver employment, and available hours depend on Health First Colorado requirements, the member’s assessment and care plan, the Case Management Agency, and applicable agency standards. Leon Home Care does not determine Medicaid eligibility or guarantee authorization.
